Case Manager Jiahui Concierge Health Program

Clinical Reporting to Director of Medical Director of Concierge Services
Administratively Report to: Chief Clinical Officer – Dr. David Cook
The qualified individual is responsible to support the development, implementation and, delivery of Jiahui concierge services and personalized health management program for concierge clients.
Responsibilities:

Support the development and implementation of a personalized Health Management Program as a concierge service to select clients and their families
Optimize the client journey through services at Jiahui Hospital including:
a. annual health assessment
b. cancer and major disease risk assessment
c. advanced diagnostic testing
d. international 2nd opinion and/or medical referral
e. longitudinal health management
Work with a limited and select panel of clients and their families, informing them about their health, educating them to make them better prepared to make health and lifestyle decisions.
Promote “P4” medicine, an approach to medicine that is Predictive, Preventive, Personalized and Participatory.
Work with the HMP team, nurses, client service managers, and other clinical services to develop and implement personalized, longitudinal health management plans
Use a team-based approach to meet the health goals of client and their families by addressing acute health needs and optimizing long-term wellness
Collaborate with the Jiahui Wellness center to support client needs for services such as weight management, smoking cessation, diabetes management, and cardiovascular risk
Support the development and implementation of digital health services for clients of the HMP program using mobile and web-services.
Work with technology partners within, and outside of Jiahui, to optimize the value of diagnostic and health information to clients.
Develop a working knowledge of precision medicine and the application of advanced techniques such as genomics, biologic and behavioral profiling to understand and manage health risks in collaboration with clients and their families
Qualifications
1. Registered nursing degree
3. Minimum 5 years’ experience in clinical services
4. Fluency in Mandarin Required, fluency in English preferred.
